ID Required

Please view the details on the registration packs in regard to the ID required for online access (this has to be seen in person, we dont accept copies).

Two forms of ID are required for adults, one of these must contain your address e.g., tenancy agreement or bank statement and the other should be photographic ID such as a driving licence or passport. If you do not have photographic ID, then a birth certificate is acceptable. Adults should present their own ID. Children require a Birth Certificate or Passport. If none of the above are provided then online access wont be granted until this is complete. This will not stop us from registering you with the Practice.

We hope to register you within a week; however, if you need an appointment urgently, please call reception.

You will be allocated a Named GP but you are free to make appointments with any of our other GPs if you prefer.

Repeat Medications

If you are currently taking any Repeat Medications please provide your repeat medication slip from your previous surgery when registering so that they can be reviewed by our pharmacist or a GP. You may be asked to make an appointment for a new patient health check if needed before medication can be supplied. If you do not have this slip then please bring in the medication box/bottle showing your name, dose and date of supply.

If you are unwell while away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local GP practice by registering as a temporary patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. 

To register as a temporary patient simply contact the local practice you wish to use. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Blind/Partially Sighted

If you or family members are blind or partially sighted we can give you a large print of our practice leaflet upon request. Please ask Reception for further information.

Guide Dogs

Guide dogs are welcome at the surgery but we ask that you be aware of other patients and staff who may have an allergy or fear of dogs.
