If you would like to
register with the practice please
contact the
appropriate surgery
(usually the closest to your
address) and take your medical card,
if you have one.You will be asked
to fill in a registration and a ‘New
Patient Details’ form.

before attending the surgery please

to download and print the form.
However, you will also need to
complete the NHS Family Doctor
Registration form, upon arrival at
the surgery. This is a legal
document which enables the Health
Authority to allocate you to our
practice and requires your
signature, or your signature on
behalf of the person you are
registering. You will be registered
with the practice and not a specific
doctor, but you are welcome to make
appointments with a doctor of your
choice.
We will then ask
you to make an appointment for a
simple health check, with one of the
practice nurses.
